Notes:
1. CTM will provide the following equipment to customers upon successful installation (one line for one set of equipment only)

  • One Broadband Modem with Ethernet LAN Cable and Power Adapter
  • One Microfilter (only applicable to customers using existing telephone line)
2. The above equipment shall be returned in good condition upon termination of service. Otherwise, fees will be applied according to clause 18.1 of the Terms & Conditions.

Deposit

For the Broadband Service Deposit Payment, Customer must sign the "CTM Broadband Service Deposit Agreement" once the Broadband Service has been installed. In addition, Customer is required to approach any CTM Shop for the payment of the corresponding deposit before the printed due date of the first bill.

Provided that the Customer who had subscribed a CTM contract service (including Broadband Service, Fixed-line Service or Mobile Service) for at least over one year and the service had not been terminated due to overdue payment or a bad debt record, then the Deposit for the Broadband Service subscription will be waived.

Declaration
  1. I / We certify that the above information is correct and agree to have the necessary computer accessories (including the Network Interface Card) properly installed prior to the on-site installation provided by CTM.
  2. If I (Customer) wish to terminate this Agreement, then I shall notify CTM in person by written request at any CTM Shop as I understand that the No. 1 Hotline:  1000 will not accept any termination requests.
  3. I agree to have the invoice paid within 20 days from the issue date.  Failure to do so may result in the suspension of Internet Broadband Service and CTM reserves the right to pursue legal actions in respect to any late payments.  During the suspension of service, the respective monthly usage charges will continue to be levied and CTM shall only reconnect the provision of the Service against payment of a reconnection fee and until I (Customer) have settled all outstanding payments.
  4. I agree and accept to pay for the Internet Service monthly charge in advance, for the current and subsequent month.  The additional usage charge will only be charged in the following bill.
  5. I agree to be bound by the Terms and Conditions of the Services provided by CTM. Details please refer to http://www.cyberctm.com.
  6. I certify that the above information is correct and I have read and accept to be bound by the "General Terms & Conditions of Internet Services".
  7. I agree that “E-mail Bill” which I shall use and is the way for receiving the bill.
  8. I agree to protect and secure the User ID and password that is provided by CTM at all times.
  9. I agree that if the services I opt to "E-mail Bill" is a Multi-bill Account, then all services under that Account will be grouped as one E-mail bill.
  10. I understand and accept there is a limited storage capacity of my E-mail account for the "E-mail Bill" service.
